As a home care customer of Holland Medical Equipment,
Inc., you have the rights to:

Select those who provide your home care services.

Be treated with dignity and respect without regard to
race, color, creed, sex, age, national or ethnic origin, diagnosis, or
source of payment.

Be provided with information regarding ownership,
available services and charges.

Be instructed in the care and use of equipment in
order to reach the highest level of self-care and wellness.

Continuity of care and service provided by personnel
who are qualified through education and experience to perform the
service for which they are responsible.

Participate in transfer/discharge process, or referral
to another level of care.

Be informed within a reasonable time of anticipated
termination of service.

Voice complaint or grievance without fear of
discrimination or coercion, and be informed of procedure for
registering complaints.

Receive a timely response when home care equipment is
needed or requested.

Receive an explanation of all forms you are requested
to sign.

Receive home care equipment regardless of race,
religion, political belief, sex, social status, age, sexual
preference, handicap, or any other classification protected by law.

Have all your records and all communications, written
or oral, treated confidential.

Have access to all health records pertaining to you.

To formulate and have honored an advance directive
such as a Living Will or a Durable Power of Attorney for Health Care.

To be involved, as appropriate, in discussions and
resolutions of conflicts and ethical issues related to your care.

To have privacy and property respected at all times.
